Whenever I use a 3D program, at random points, the editing viewport goes black, and the software hangs. The GPU load histogram shows a spike and then 0% load.
Everything else runs fine, I can switch to task mgr and end process, and GPU load histogram shows standard load pattern again.
What causes this? My drivers are updated.

To answer the question specifically, it would be necessary to know the specific hardware and software used.
My guess is that you may be using a GTX or Radeon which are optimized for frame speeds. Complete 3D CAD viewport support requires the special drivers developed for workstation GPU's- Quadros and Firepros. These drivers are optimized for OpenGL. In tests of Solidworks, a $250 Quadro will run viewports faster than a GTX Titan.
Have a look at the sites for the software you're using and/or find lists of recommended graphics cards for that specific use.

If you need a GPU to run viewport CAD, consider the recent line of Quadro Pascals, especially, P1000, P2000, and P4000. I was an early purchaser of a P2000 5GB ($420) and on my system it produces a Passmark 3D score of
9012, which is above the average score of a GTX Titan Black (8567) and GTX 1060 6GB (8719). The P1000 ($330) runs quite a bit faster than my previous Quadro K4200 that cost $850. The P4000 ($820) is the new "VR-ready" model and on Passmark is faster (10388) than the M6000 24GB (10091) that cost $4,000 new.
