I believe they are identical with the exception that the Extreme model has a significantly higher factory overclock at 1253 MHz. That is probably among the highest factory overclocks you can get on any GTX 980 Ti. It also has a rare overclock on the memory. The Omega edition has a factory overclock of 1178 MHz and no memory overclock. Otherwise they should be identical in terms of cooling and components.
