why my graphic card core clock is stuck at 405 MHz?

My pc was not responding so i had to turn of the pc from the main switch,
from that day onwards something happened to my graphic card it's core clock was stuck at 405 MHz. I would be glad if someone could help me with this problem please!!!
Here are my pc specs

intel I7-3770k
8GB DDR3 memory
gtx 680

Have your tried uninstalling the GPU drivers with DDU and reinstalling them?
I'd start there first.
