That's what about dot com told me. It seems that your hacker is guising himself as a security code. I would try starting everything fresh, dropping all online payment methods, do everthing by mail, and set up all new accounts, and make sure that you never use your old accounts, on your new computer. You could try switching operating systems, to either mac or ubuntu, or even chrome. (assuming you're on windows.) At the very least, bring it in to a computer shop so that they can help(no, not best buy or somewhere wher they hire people at min wage to say "well, this is a software that may help" and give you norton antivirus). Find someone who knows computers, and get their proffessional help, for a few hundred bucks.

One of the best things to do when having hacking problems is to format your machine and start over. Back up any .jpegs or your old tax returns, but save as little as possible. (they may have a virus in a .pdf or something similar.) Format the disk so EVERYTHING is gone then reinstall windows. Make sure you update it fully and have an antivirus on it. (AVG, and Aviria are two free ones.) It should be a lot harder for anyone to get in now.

Building on what 4745454b has said, when you format your hard drive disconnect from the internet and clean the drives completely. If you have any pictures or files that you must save put them on a USB stick.
Get a new hard drive and a new copy of Windows and load your computer with the new OS. Get a new copy of anti-virus software and load that after Windows.
Before connecting to the internet unplug the router and the modem and wait for at least 10 min before pluging in the modem, then do the router and set up your network.

In Windows accessories there is a virtual keyboard, keep that in the task bar and use it to input any passwords. NEVER input ANY passwords unless your using that virtual keyboard. The reason for this is so that any keylogger will only see click,click,click and no letters or numbers. Keep your passwords strong by using a combination of numbers , letters and upper and lower case letters and symbols where allowed.

If you have loaded a USB stick with saved photos or documents let your antivirus scan it when you plug it back in.
