Solution
The proc can still do the job but The GPU is too weak if you are aiming for AAA games on 1080p high/ultra settings @ around 60fps.
I would go for GTX970/R9 390 or higher.
The thing is, your 450W PSU is too weak for those GPUs, you have to replace your PSU with something around 500-550W.

If you want playable frame rates at higher than low or medium settings, you're definitely going to want a higher end GPU card and PSU. That's a fairly underwhelming power supply that probably shouldn't even be tier 3, but is, which is still not on par with what we like to see used with high end gaming cards.

A Tier 1 or Tier 2 unit would be a serious improvement, even with your current graphics card.

You ask a really subjective question. Lets take an example, Witcher 3 is a current gen game and is hard on even 980, 980 Ti cards. The next gen games will up the ante even more. Plus with DX12 now in play we should start to see some awesome games. Of course you can always scale back the games graphics to play it on a lower end card but with the top bar being moved so much higher that means a greater sacrifice to play the game on slower card.

What about Medium at 1080 ? i got pretty good cpu and decent gpu man...

The CPU is three generations old. The GPU is low end for a gaming system. For games that are two generations old it will do fairly well. For current and future gen games, not so much. You might, probably, make medium settings in many titles but for very demanding titles like Witcher 3, GTA V, or upcoming titles like Doom, Fallout 4 and whatever else comes out going forward, you're going to see a significant lack of the necessary resources.

you seem like you trashtalking man, i ran gta 5 on high settings x2AA 1920*1080 with 30/40 fps same goes for Witcher 3 ...
