Win 10 Display Keeps Losing Signal

Whenever I do something like play a game of league of legends I am able to load into a game and walk around for like 20-30 seconds but then my monitor goes off and says "no signal". My computer power still stays on but I am unable to get the display on without restarting.
One thing I noticed is that sometimes the computer restarts itself but when it does, I notice the resolution is lowered, it goes to the loading windows screen with the blue flag until it freezes. At that point I let it sit a little bit and then eventually restart for the resolution to go back to normal. Ive updated to the latest drivers for my graphics card and I have the latest windows updates. Also, at first it was also doing this when just sitting in the desktop. Until one of the restarts, a windows update started and then it stopped doing it on the desktop.

CPU: I5-3550P
GPU: Galaxy GTX 560 2GB
PSU: EVGA 500B
OS: Win 10 Home
 
Hi,

Do a clean install of the graphics card driver of your PC.
- Go to Device Manager and uninstall the Nvidia GTX driver.
- Check Programs and Features and uninstall anything related to Nvidia.
- Once done, download and install the latest graphics driver.
- Here's the link: http://www.nvidia.com/Download/index.aspx
- Reboot your PC once the latest driver has been installed.
- After the reboot make sure to set Nvidia as the default card.
