Migrated my system to a new build and the system keeps on crashing with the exact error.

First I thought it was a RAM issue so I replaced all of them then I replaced the motherboard. OS was reinstalled at least 3 times at this point using different images.

OS: Win 10 Pro
System Spec
CPU: i7-6700
MB: Gigabyte B250M-D3H
RAM: 32Gb PNY Anarchy MD16GK2D4213315AB

Below is the crash dumb log

Solution
does it still crash when only the native HDMI cable is in use and the DP unplugged? How about the other way around?

How are you converting the DP signal to HDMI exactly? a standard converter cable or an actual active converter?
CPU Temp 72F, load is only 5-10%

On my 2nd build, I let Windows take care of all the updates and left it barebone to see if it crashes which it does.

This last reinstall of the OS, I updated all the latest drivers from Gigabyte site with the exception of the intel driver as on the recent built I used the one on the GB site with the same error. I've isolated that it's not the motherboard because I started this new built with an MSI B250m Mortar Artic board with the same error.

This is not a gaming computer, it's my NVR so I have nothing but a mouse 2 monitors and a speaker hooked up

I don't have any other GPU on the system at this time just the integrated one. But I do have an available HD8750. I can disable the integrated GPUand use just the the external GPU and see if it still crashes.

However that still doesn't have my issue. Paid extra attention on selecting MB so I can use the integrated GPU with HDMI, DP and a DVI so I can use all 3 monitors without a discrete GPU so I hope to solve this.

The DGPU is simply a test, It's quite LIKELY that while the system is crashing in the Intel driver, the fault is more likely to be something common between all your attempts... like monitor cabling causing a fault (or if you had one a webcam or other video input device)

Which brings up the next point, what exactly are the monitors and how is each connected

A little update on this.... it's the weirdest thing, I think the DisplayPort is the one causing the issue.

Since I removed the monitor on the DP I have no more issue, Tried using another DP and the issue came back.

I installed the HD8570 video card ( using the same DP adapter and been using dual monitors with no problem.

Any reason why would it be doing this? Not only on this motherboard but the other board as well.

I decided to use 2 DP adapters on the same 2 monitors on both an external and the internal card. No problem in the last couple days.

So there's an issue with the internal card driving dual monitors.

I updated the MB to the latest Bios last week upon having the same issue. The IGPU is either AUTO, ENABLE, OR DISABLE. So it's either on or off no additional settings for dual monitors.

Very strange to me as well, both new motherboards a GB and an MSI have same symptons.

The HP machine the CPU came out of has dual HDMI and I never have issues.

Instead of fighting the problem, I ordered a "cheaper' MB with only a single HDMI port, I'm going to use an external card to drive 2 monitors 1 with a DVI to HDMI adapter and another with a DP port. I'll still get my 3 monitors when it's all said and done.

even the lowest Discrete graphics cards will drive three monitors, that's the true benefit to them. I don't even try with the Intel GPU's anymore, on the office Dell and HP's I just stick GT 1030's in them and call it a day after disabling the onboard.

Annoying
