I had that issue when I upgraded. I had to reinstall and it went away.

I fix it buy only going to the safe mode and doing a restart again.
If it doesn't helped you , as SinxarKnights said , do a reinstall

you can get the media creation tool from http://www.microsoft.com/en-ca/software-download/windows10 just follow the instructions on the page and you should be fine hope this helps
