tested memory for 24 hours = OK
ran DISM and sfc /scannow = always a problem
reinstalled Windows 10 pro x64 multiple times
everything was new and recently purchased
can't stop the freezing, screen looking like a Sci Fi movie (flipping and out of control)
Some times the sound goes berserk as well...

Out of ideas my only guess is the motherboard is bad

I just ordered a GIGABYTE B360M DS3H (LGA1151/Intel/Micro ATX/USB 3.1 Gen 1 (USB3.0) Type A/ DDR4/Motherboard) and will replace ASRock - H310M-HDV/M.2 Micro ATX LGA1151 Motherboard

Have you tried updating drivers and BIOS after you reinstalled OS?
Have you noticed that freezes/BSOD occur when running something specific in OS?
Any OC to memory that you've made?

Please upload dump files located in C:\Windows\Minidump

I have tried all of the above suggestions

Update all possible drivers
Uninstall all unused programs
Run chkdsk /r /f and sfc /scannow
Perform a clean boot: http://support.microsoft.com/kb/929135
Disable temporary all security softwares you have
Run memtest86+ to check your RAM. If an error was detected then replace the faulty RAM or contact your manufacturer Technical Support for assistance

NVIDIA GeForce GT 430 latest drivers (I realize this video card is old but with this mobo it seems like the only way I could duplicate my PC monitor to play on a Samsung smart TV and watch movies saved to NAS)

Have you tried updating drivers and BIOS after you reinstalled OS? yes all are latest
Have you noticed that freezes/BSOD occur when running something specific in OS? seems to be at random
Any OC to memory that you've made? no overclocking period

OK so it seems that your GT 430 is the main issue, although you updated drivers sounds like this video card is faulty,

but I would recommend doing some diagnosis to verify that. Try to plug this GT430 to other PC, just to check if freezes and crashes occur again -> if it does then your GPU is faulty.

Instead of you I would disconnect this graphics card out of your PC, it probably won't freeze or crash using your onboard graphics card (it does not seems that your motherboard causing the issue)

Although I still believe it is a driver issue like you have determined. The system had blue screen issues with the Intel® UHD Graphics 630 as well. That is the main reason I tried the NVIDIA GeForce GT 430 (which was already in my possession)
I was fighting with blue screens before I added the discrete card.

I appreciate all the work that you have done. In the near future I will try to swap the ASRock mobo for the GIGABYTE B360M DS3H (LGA1151/Intel/Micro ATX/USB 3.1 Gen 1 (USB3.0) Type A/ DDR4/GIGABYTE B360M DS3H (LGA1151/Intel/Micro ATX/USB 3.1 Gen 1 (USB3.0) Type A/ DDR4/. I am recovering from a broken hip and upper femur from a bad fall and last night I slipped and broke my Clavicle -- also called simply broken collarbone and pain is excruciating.

One last request could post or direct me how to find the debug results so I can study the results in greater detail.

thanks again for all of your help

Indeed, I would RMA your motherboard if the problem occur without the NVIDIA card.

dump file debug was made with WinDbg64, Here is the full debug log:
