Being that you can see it up top, it seems like this is a context menu issue. I would recommend that you do a system restore. It's strange that it is happening out of the blue.
If you have hold down shift and right click, is it there? Also, when in the folder, in the top portion, is there an option that says Open with an arrow?

If it's one specific file type, it takes about 10 seconds to change the default file association in the control panel. If there's a problem with the registry that broke the context menu, there are fixes for that, but doing a system restore is a shot in the dark as to when it stopped working.
