Read the above links for how to transfer the license and activate it on your new hardware.

You create your own Windows 10 install DVD or USB.
Here: https://www.microsoft.com/en-us/software-download/windows10

Then, do a clean install on the new hardware:

Not the way you're thinking. no.

Going into a new PC, you really really need to do a full reinstall.
What OS is this? If Win10, you can probably move the license. Where did this OS come from?

But the actual OS will probably need a full reinstall.

OK, for that, you can transfer the license, and you don't have to buy a new license for your new PC.

Read and do this before you change any parts:
https://support.microsoft.com/en-us/help/20530/windows-10-reactivating-after-hardware-change
http://www.tomshardware.com/forum/id-3164428/windows-build-1607-activation.html

For the actual installed OS, there are 3 possible outcomes in moving the drive like that.
1. It boots up just fine
2. It fails completely
3. It boots up, but you discover issues for weeks/months after.

For a whole new system, a clean install is strongly recommended, often required.
