C and D partitions had swapped letters
So the old D now became C and the old C (the windows drive) is now D

I tried to revert this through cmd > diskpart
By assigning a different letter to the current C (old D) and assigning the windows files drive back to C

And it tells me that the letters have been assigned successfully, but as soon as I reboot my PC they are swapped again
And there is no way I can log into my windows now, any help is appreciated

*the disk is GPT and my motherboard supports UEFI, I used to log into this drive without any problems

Moderator Edit: Moving post from Power Supplies to Storage

I'm pretty sure I posted that in Windows 10 section

"Windows 10 section" - likely so and I may well have mistakenly erred in my edit. Apologies.

Do the following :

In the 'Type here to search" box type "Create and format hard disk partitions".

Capture the full Disk Management window that opens and post via imgur.
