I've been playing around with the settings on Witcher 3, but I can't get it to run smooth. I am running 12GB of RAM with a quad-core i7-3770 processor and Geforce gt 640 video card with Windows 8.1. Please give me some suggestions.

your video card isn't good enough for witcher 3.
It's not meant for gaming, you'll need to spend about $150 if you want one that can actually run witcher 3.

Part of the issue is your GPU and I have had some choppiness off and on due to some forms of glitches. I sometimes need to reload the game to get rid of the issue if it persists for more than a few minutes. Best thing to try for now is turning settings all the way done to see it helps and make sure the game and drivers are updated.
That GPU though seems to be your reason for issues.
