The Toshiba i7 computer has an Intel hd graphics 4000... Can it play games like the new assassins creed blackflag and is it possible to change the graphics card even if its Intel

Intel HD 4000 is not an actual video card, it's part of the i7 CPU that is dedicated to video. It won't run very well. You would have to play on lowest settings, plus a low resolution probably below 720p.

You could add a video card, as long as the PSU has enough power to run the video card. You might have to upgrade the PSU depending on how good of a video card you want to buy. If it's a laptop it's more tricky to add a video card.
No, integrated graphics are far too weak to be able to run high-end games at reasonable pace.

And you can't change the graphics BECAUSE it's integrated into the processing chip. The only way to boost graphics is to add a discrete graphics card. And I'm assuming you're using a laptop; so before you ask, no, it's not possible to add a graphics card.
