The guidebook mentions that Khelly's dagger stats may vary slightly. It
says: "The exact stats of the eternium dagger may vary. A recent report...
revealed the dagger may be as good as (+0, 2d4+10)"

To me, this implies the dagger may not follow the same set of generation
rules as other items. That is, (+0, 2d4+10) is the best dagger ever
reported.
Is there any evidence that his dagger follows any rule other than what we
might find from a randomly dropped eternium dagger? (This evening, he
dropped an eternium dagger of slaughtering (+0, 3d6+4))

No, there is no evidence that his dagger follows any other rule. What
version introduced the ego weapons? That is a serious deficiency in
Appendix N of the GB.

Thanks Markku. On further perusal, I see I did mention item prefixes
and suffixes in 1.0.0 but did not use the words "ego weapons".
