If you have gone through the companion quests to become a werewolf, and decide it isn't for you and you would like to cure yourself of the curse of lycanthropy, you do have an option to do so.
Step 1)Begin the quest Purity, offered by either Farkas or Vilkas.
In order to be offered this quest, you will need to have a Glenmoril Witch's head in your inventory and done at least one other random quest for the quest giver. This can be obtained by completing the quest 'Blood's Honor', which is also available from Kodlak after doing some other companion related quests.
Step 2)Bring the quest giver to the flame of the harbringer.
This is located within Ysgramor's Tomb, which you would have already visited during the Blood's Honor quest. Simply approach the flame and burn the Glenmoril Witch's head in the flames to summon a wolf spirit. Defeat this and you will have cured your companion of his curse.
Step 3)Cure yourself as well.
Now that your companion is cured, approach the flame yourself and throw another head into the flames to bring forth your own wolf spirit to be defeated. Both spirits are pretty straightforward and easy to defeat. Bring a cure disease potion with you just in case though, they can afflict you with rockjoint.
Once the wolf is dead, you will now be permanently cured of your lycanthropy, and there is no way back from that. You are permanently a human. Unless of course, you decide to become a vampire instead. If you have the dawnguard expansion and agree to become a vampire lord while you are also a werewolf, you will also cure yourself of lycanthropy, while infecting yourself with vampirism.
