Windows 10 was the culprit, i downgraded to Windows 7 everything is running super fine now..

UPDATE: I downgraded/rolled back Windows 10 to Windows 7 all of my problems went away, i was getting less ram usage, less CPU usage and less HDD/Disk usage, which made my PC run quite fast and games smooth but i will still add more ram to my computer. :)

Thank you guys for the suggestions i will let you know when i buy and add another 8gb(2x4).
