So I just had a similar issue. You can either use a custom resolution or on the PC resolutions page scrolls down to PC underneath the typical resolutions (UHD,HD,SD) and see if the refresh rate you want is under one of those resolutions.

Are you using a VGA adapter on one of the DVIs? Try disconnecting one of the existing two monitors and see if it allows your new monitor to become extended. If so you will need an Active displayport connection for the third monitor.

I've always understood the rule as that anything over two connections (VGA,HDMI,DVI) require and active displayport adapter or native displayport. So if you already have three connections and one is using displayport already the fourth would need an active adapter or native displayport.

This is just a assumption but you're using HDMI in and out on the Yamaha it's likely it's dropping the video signal from the PS4 before it comes to the monitor.

How many monitors are showing up currently? I've done a ton of 6 display trading machines and it really does make life easier to just do native displayport monitors.
