Indeed it is rare. A friend of mine has an R9 390 which tended to reach 90 degrees till it began to throttle. He had to put on a G10 bracket with an H60 AIO to keep it below 70 degrees!

I previously had a Pentium G3258 at 4.2GHz paired with an MSI B85M-P32 motherboard. Everything was all good and stable too (at 1.28V). Also, I had an R9 380 Gaming 4G which was quite a hot card. After switching out the motherboard for an ASUS H81M-PLUS, I can get the same Pentium chip to 4.8GHz...

Case fan placement can get a bit complicated. The simplest arrangement is to try and get a neutral pressure in your case, by having equal intake and exhausts. 1 in the front and 1 in the back is fine for less power-hungry builds. Your friend must've been talking about PWM fans, which use 4 pins...
