I'm having the exact same problem. Recently upgraded to Win 10 (Due to memory limits on Win 7). It was a clean install. I have my 3 monitors bound together via Nvidia Surround (5760x1080). But when I tick the box to add my 4th monitor (My TV) it just fades out and doesn't do anything...

Yeah try resetting the CMOS (Removing the battery on the MB and replacing). You'll lose any overclocking/custom profiles/date/time. And try changing the the SATA port on your MB. If you have SSD/HDD/Optical Drive. You want the SSD in the first port, then optical drive in the next, then your HDD...
