Well, this game is poorly optimized for amd gpu's, and possibly might be the issue. Try disabling MSAA, since amd cards never run that well lol. Are you getting lag in other games that are not open world?

It's your cpu. An open-world game mostly relies on cpu, therefore it must be the problem, as it is aging and losing its capabilities.
