I as well have a medion pc with the H81H3-EM2 motherboard. Further i have: i5-4460, 8gb ddr3 ram, GTX 750 1GB, 350 watt PSU. I also want to upgrade my Graphics card but then i need a new PSU. Is it possible to do this with the 14pin connector on the motherboard?
Well I decided to go for the safe option and ordered a new motherboard as well.. Didn't want to risk frying stuff which can happen with such a cable.

Sidenote*: A new psu is probably needed anyway since you want a proper gpu i suppose, and in that case it's the best option to go for a newer motherboard as well (My opinion)

Well, I still have the case that came delivered with it, so yeah that fits. It does take some time though, to replace PSU and motherboard. (You will need to reapply thermal paste when you switch motherboard). I'm not a master at pc's and was able to do it myself. If you take your time it's quite an easy process. Just get your old PSU and motherboard out of the case(remember which cables sticked where). Keep CPU & RAM on your motherboard (that's what I did) then screw in the new PSU. After that place your CPU + Cooler and RAM on the new motherboard. Screw your motherboard in and start plugging things into the motherboard, most often if something doesn't fit it shouldn't be placed there. Look in the guide of the new motherboard where your pwr switch cables etc need to go. Fit those as well. Fit in the new GPU, and you're ready to go. You might however lose the front two usb's since they're all weird in my case. They litterally put USB ports on the motherboard and then wired to the front of my case.
