760 Not being detected *anymore*

I have a gtx 760 that I was using as my primary video card. It's been in use for about 6 months and stopped working the other night.

What I have done:
- Tested two known working PSUs with it. (The fans spin on both, computer is able to boot with both, etc.)
- Tried it in 3 different PCI-e slots on the same board. [None worked.]
- Reinstalled the SM Bus, uninstalled the graphics driver, attempted to reinstall (after all of the above) - doesn't recognize card is in the slot at all.
- Checked my bios settings, both before and after attempted reinstall.

What I have not done and cannot do:
- Test it in another computer. [None available.]
- Verified that all 3 of my slots didn't simultaneously die. [I don't have a reader, but I'm pretty confident this isn't the case.]

Additional information:
It stopped working in a weird way. It wouldn't get recognized so I reinstalled the SM bus and restarted and the card worked, then I ran a game, computer froze and crashed, and then I couldn't get the device manager to recognize the card let alone get a display off of it.
The fans work, but all that means is power is going to the card properly.

Win7 64-bit
Asus p8z77- LK mobo
Card life span: 6 months working
EVGA 3-fan model.
Changes in software: I installed FFXIV 2 days before it stopped working. 2 years ago there were reported card burnouts, potentially related.

Let me know if I left anything off.
 
It might have burned out according to my other thread (haha, why the other thread has answers and not this one, I don't know.)

It generally ran between 50-60 C so I don't know.
