A dying graphics card?

So I recently having some unusual symptoms it mainly happens while playing games, the game freezes and I can't continue and when that happens I face one of the two things below :

1- I get a BSOD so I force restart
2- The game freeze but I can close it through the task manager and I can explore windows normally BUT I get like some artifacts while using some programs and games etc ... and i have to restart also to get rid of these artifacts

Am I experiencing a dying GPU ?

My built
AMD Phenom II X4 965
ATI Radeon HD 5770 (40C idle , 70-80C while gaming)
8 GB RAM
GA-78LMT-S2P

That looks like vram failing, try switching to integrated GPU, if that helps you known its GPU problem.

Another problem could be with your PSU, what psu you got dude?
