My ASUS GTX 660 is constantly crashing and it is driving me crazy!
Here is the quick background:
• Bought ASUS GTX 660 on Black Friday 2013
• Worked for a month or so and then started crashing in all Steam, Origin and UPlay games
• Turned on PC one day and the screen was completely messed. See pic

• I RMA’d Video Card back to ASUS (process was time consuming and cost me an additional $35 in shipping and a month of no video card)
• Received a brand new ASUS GTX 660
• I reformatted entire computer and installed new GTX 660 with latest drivers
• SAME ISSUES ARE OCCURING with this brand new card. It is constantly crashing while trying to play games from Steam, Origin or Uplay (Civ 5, Assissins Creed 4, Mass Effect 3, Company of Heroes etc). See pics below from Mass Effect 3 game play right before it crashes.

Notes:
All drivers and windows update are up to date.
I used FurMark to test my GTX 660 and it worked fine for 40 minutes straight at 100% strain. WTF!!?
So to troubleshoot I borrowed my buddies EVGA GTX 670 to test on my system to make sure it wasn’t my PSU, MOBO or RAM etc.. and of course his card worked perfectly fine playing all games correctly in the same PCI slot and using the same cables.
The main difference between my GTX 660 and my buddies GTX 670 is that mine takes a single 6 pin PCI power connector while his takes 2. However the fact that his card worked fine leads me to believe that my PSU is fine as well……
Any suggestions? The amount of time I have spent trying to fix this issue is ridiculous. Any feedback is appreciated!

CPU: Intel i5-3570K (Not overclocked)
PSU: Ultra LSP 650 Pro
MOBO: GigaByte GA-Z77-DS3H
RAM: ADATA 8GB DDR3 (2x4GB)
Storage: Intel SSD 300 Series 240GB, 1TB Seagate 7200rpm
