Asus mobo RMA Checklist PDF?

Trying to RMA an Asus board, basically given up on this happening in a reasonable timeframe...

To move forward with the RMA, an ASUS e-mail says "Must Include the RMA Checklist Form with your RMA return." but fails to attach such a form.

Indeed, it seems impossible to find the form on their website or with extensive google searching.

If anyone has seen this form (has a PDF/printable copy), or has done a successful MOBO RMA without one in the last few months, I would like to know about it. (especially if it seems to be just a ploy to eat up time before the 30 day RMA deadline.

I simply googled asus rma pdf and it was the second link available. Hopefully that's all you need unless I'm confusing it with what got you to this point then I apologize but hopefully this helps you resolve your rma process asap. Good luck!

UPDATE: I did some digging and found this but not sure who or what supertalent is:

I don't think that's what I need, I found this:

which indicates a specific "RMA Checklist Form" to list components being shipped. I would like to find a copy of that document that I can print and submit with my mobo.
