I currently have an Avermedia Live Gamer HD and I want to be able to hook my Ps4/Xbox One to it so I can stream on Twitch using OBS. I'm using a three monitor setup and would like to use my center monitor for my game capture. Do I need to hook an HDMI to the cature card and then to the gpu or can i just run the console to the in port and the monitor to the out port?

Console's HDMI -> Capture device -> Monitor. You video card plays no part in this type of capture[If you are capturing PC gameplay, your video card would connect to this device instead of the console], the capture device does all the work and just sends a video steam(compressed most times) to the computer.

If your computer has multiple inputs you will be able to switch from computer to game console.

These device work this way(pass thru) to avoid video lag when recording(the stream as seen on the computer[in your software] will still have a delay).

Normally users would go
Console HDMI(or component) -> Capture device -> TV and the computer would just do the streaming while they play on TV.
