Black Flashing Squares using R9 280x

Hi all

Having a problem as shown in the video below, I hope lol ANyone know what may be causing this. It seems to happen mainly on facebook and such when embedded videos are present.

It's usually due to unstable OC on CPU/GPU, bad graphics card ram or a driver issue.
