Black screen when gaming then screen out of Resolution

Hello guys. i need your help. for some tine now while playing games like Cod ghosts, my pc crashes, doesnt completely but everything is frozen. when i restart the pc l, the screen is out of resolution.

i have an asrock bios h110m-fgs/d3
i5 processor 12gb ram being a 4gb and 8gb hyperx ram
gxt 970 graphics card
im using my led tv as a monitor.

At times id still experience an out of resolution screen when booting the pc and takes time to be fixed

my screen will go black while gaming but not completly restart. when i manually restart, the screen will be out of resolution and difficult to fix again

i have a asrock h110m board
