Can't OC FX-8350

I cant' OC my FX-8350 CPU even by 8%. I end up getting BSOD. Adjusting the voltage does seem to help but even the smallest overclock will inevitably give me a BSOD.

During Prime95, temps are noticeably higher however still well under the suggested limit. I'm using a 212 EVO aftermarket cooler for my CPU.

I have also made sure all the "ECO/Energy Savings" settings are turned off on my mobo.

This was the first test I did without modifying voltage
4200MHz (210x20, 5%)
Prime95: 5 min, 1 error
65c Socket, 52c Core

With this, I could get around 30 mins of game time before BSOD
4300MHz (215x20, 7.5%), Voltage +.15
Prime95: 11 mins, 0 fails
Max temps: 71c Socket, 56c Core

I'd recommend taking a look at an FX overclocking guide to get more familiarised with it. I generally prefer not to use voltage offsets and instead set a value manually as well. The instability may not just be due to the CPU clock and core voltage, so please do also check your CPU/NB, memory and HT to make sure that everything is ok.
Also, you seem to be overclocking by increasing the bus speed rather than the multiplier. Is there any reason you are doing that? The 8350 is unlocked so keeping the bus at 200 and just adjusting the multipliers will be much easier.
 
What psu do you have? How many watts? How old is it? For FX 8350s most like to have a 1000 Watt 80+ psu as they tend to be power hungry. Also psu's loose effectiveness over time (like we all do) so if yours was 1000 Watts five years ago it isn't 1000 Watts today.
