You'll have to excuse my complete dumbness when it comes to this.
I brought my first PC last year, and I only got a small, less demanding one (AMD Radeon HD 8470D with an A6-6400K APU)
However, I would really like to upgrade my computer but, I don't know how the transition begins (e.g what one to get, if I need to do anything extra if I wanted to change from AMD to Nvidia card etc etc)
Is it possible for anyone to give me a guide to upgrading?

1. Make sure your Motherboards has an PCIe x16 slots
2. Make sure you have a decent Power Supply, a non-quality PSU is not recommended
3. Make sure you have the GPU card itself (joke :D)
4. Using an AMD Chip doesnt affect your GPU at all. For example you are using an APU A8-7600 and looking for dedicated GPU like a GTX 750ti, then you just install them on your motherboard and have no problem at all.

for your weaker dual core cpu max gpu you should get is a rx 460

you need to make sure you have at least a half way decent 350w power supply
