Changing Resolution Sometimes Causes Monitor To Go Fuzzy

This has been going on for sometime now (A few months) and I have no idea how to fix it.
Typically when I'm playing CSGO I'll change from 4:3 to a 16:9 aspect ratio, changing the resolution as well. But sometimes my monitor will come back, appearing as fuzzy, glitched out, like someone just super-man punched my screen. Unplugging the monitor and plugging it back in causes the monitor to not be detected, and the computer has to be force restarted because the monitor can become normal again. I should have taken a photograph but I did have the chance, based on what I asked though does someone have any idea?

usually that happens when you're using an hdmi ? if you use dvi or vga shouldn't be blurry

I use VGA cable, and do it same. with pnp connect/disconnect sound.

And do it the system only from today. (win10, motherboard integrated videocard)
