Computer goes sleep randomly and freez

Since yesterday i have this problem. My monitor going to sleep mode randomly and i cant use my pc. I need to turn off my pc and turn on my pc to use it again. I built my pc in december 2015 and i never had a problem. i uninstall and install my gpu and ram but it didnt work.

Can you help me how to fix this problem?


PC Spec:

core i5 4460 cooled by Cooler Master Hyper 212 EVO
msi z97 g43
8 gb(2x4) corsair vengeance 1600 mhz ram
asus gtx 960
Cooler master Real power 520 watt power supply
120gb sandisk ssd
1tb seagate barracuda
windows 10
 
Solution
First to try: using DDU to unisatll the gpu driver in safe mode then reinstall the driver from nvidia site.

Then go into the BIOS to check the 3.3V, 5V, and 12V ( read the manual to see where you can get those reading), I think the PSU, the CM 520W, may cause the problem.

The check the gpu temp, and double check the cpu temp too, even you had the 212evo by HWiNFO64 http://www.hwinfo.com/download.php

Solution

Its my psu that causing the problem. The gpu doesnt go above 65 and cpu doesnt go above 45 so thermal isn`t the issue.
i just bought a new psu. its SeaSonic M12II EVO 520 Bronze.

It happend again. I already replaced my psu

And the problem may relate to the out-of-date drivers too,
