Hello,
I am using a Z87-A motherboard in my system and replaced my PSU with a Seasonic Focus plus Gold in the last year or so.

I recently realised that my CPU fan (cooler master evo) had stopped working. I attempted to plug it to the optional CPU fan slot on my mobo but the fan didn't work. I then proceeded to replace the Cooler master fan with the one that came with my CPU (small intel fan). The CPU FAN slot on the mobo didn't work but the optional fan slot worked.

I fiddled around with my system (regrettably, trying to reinstall the evo which would spin a few times then stop) and somehow ended up not being able to get into windows past the log on screen. I could boot past the bios, then windows loading screen but then screen would go blank and the system did nothing. It looked like a graphics error. I have now switched to intel VGA drivers of the mobo so I can get the system to boot into windows properly and this also worked which has left me wondering why my graphics card has failed after the fan failing.
My graphics card is a Sapphire R9 290 Tri-X which takes power directly by the PSU.

My question is, considering that the PSU is fairly new, would that sound like a motherboard failure?

I really hope my graphics card is alright and a new motherboard will take care of the issues.

If there isn't some fan power setting in the bios that disables the fan at low speed, yeah, it would look like a motherboard power delivery problem. Does the cpu fan work with the gpu removed in the cpu_fan header?

The 290 is a real power hog sucking up to 400w at load, 325w from the psu connectors 75w from the board, so there may be gpu errors if the 75w pci-e slot wasnt working optimally or the psu couldn't supply the remainder.

Yes, the small fan worked on both headers when the card was removed. Didn't try the cooler master as I don't want to be fiddling with the system before I can get a new motherboard.
