CPU Fan Sometimes Won't Start

My CPU fan (Hyper 212 EVO) is extremely inconsistent. Sometimes it won't start when my computer boots up and sometimes it will. Most of the time, it does not. When it does manage to start spinning though, it will stay spinning like it's suppose to. Sometimes the CPU fan will start if I "hand start" the fan by flicking the blades, and sometimes it still won't. Sometimes I can see the fan blades turn slightly after I try to hand start it like it's trying to start up, but it doesn't. Trying to control the fan speeds within the BIOS no longer does anything. I set the fan controls to 100% spin at 100% of the time in the BIOS, and the fan will stay off. A few weeks ago, setting the fan to 100% would immediately start up the fan if it was off but not anymore. If I connect the CPU fan header into one of my SYS_FAN pins slot, the CPU fan will run at a constant 1200 rpm all the time, but it would not decrease/increase depending on my system temperature, like a proper CPU_FAN pin slot, so this is not a good option. Also this is not due to my system not starting the fan due to my system being too low temp. My system has reached 60-70C and the fans will still not start. What could be the problem?
