CPU getting really hot?

Hi,
I have a i7-6800k with a h115i that I added 4 noctua nf-a14 fans on it and my cpu can't pass a stress test in OCT because it goes over 95 c. I have my cpu at 4 Ghz and 1.2v, but before I kept it at 4.2 Ghz and 1.265 v. What could cause my cpu to overheat so much? I even keep the fans at 100%, I tried remounting the cpu cooler. What should I do other than replace thermal paste?

Feel the CPU water block and feel the radiator.
The block will be hot but if the radiator is cool, your pump has died.
I don't think those can be fixed, but maybe you have a warranty and can RMA the cooler.

Hi,
I took off a fan and touched the radiator - it's warm but definitely not hot and the cpu block is warmer. In corsair link my pump is still sending readings of 3000+ rpm, and one tube is definitely warmer then the other which leads me to believe cold water is still being pumped through one while hot waters pushed through the other. I do have my newegg invoice so should I try to RMA or should I try new thermal paste? I already re-mounted the cooler.
