If my CPU is rated at 88W, how do I add up the wattages shown below? How many watts is my CPU actually using under load?

Its ab estimate. Take highest number. 88w is the tdp, not necessarily what will be used. If you want accurate readings get a kill a watt or equivalent

88W is the total maximum, not constant.

Package: Power 48.31W was the total recorded at the moment the report was taken. The lower wattages add up to 53.64W so it doesn't add up to 48.31 because the values are constantly and quickly changing.

CPU IA Cores - processor, a conventional CPU section (including floating point unit FPU) of the total power
