Diagnose freezing Windows?

Hello all,

Trying not to clutter the forums as I see this has been a repeated topic, but I can't seem to find a solution to my issues specifically. I'm having two issues which began recently(about a week ago).

1- I run dual monitors(one from HDMI, the main one using DVI) and they keep randomly shutting off almost as if the computer is begin to go into sleep mode. Generally they come right back on, but there have been instances when they have stayed off and I had to hard restart. I've tried to troubleshoot by running each of the monitors independently at a time, and it still happens. This would lead me to believe it's an issue with the video card but I'm hoping that isn't the case.

2- My second issue is Windows freezing at random moment. It doesn't seem to be an issue created by any particular piece of software. There is no consistent pattern to when it happens. Sometimes I've had to hard restart 4 times in 20 minutes, other times I'm able to work for hours without issue. I have just updated to KB4015438 and the issue is still persisting.

Is there a piece of software which can actively monitor my computer so that when it freezes and I restart, I can find out which process is causing this issue? I've ran HijackThis and found nothing. Really starting to hate this problem.
