Disk 100% usage , chkdsk stuck

Recently, when I start my pc disk usage is always between 90-100% while nothing is running.
I checked in Task manager/performance but the highest usage I around 3mb from task manager itself and still it runs between 90-100% of usage.. So I decided to run the chkdsk /f /r scan. But this has been on 10% for about an hour now. So can anyone explain to me what's going on exactly here?

After you are facing some HDD issues, It may be a good idea to back up the crucial data that is stored on the drive until you sort this out.
What is your OS? If you are on Win 8, you can try disabling several Windows Services like Superfetch, Windows Search, BITS: http://www.tomshardware.co.uk/answers/id-2027511/windows-disk-usage-100-problem.html
If the issue persists, you will have to test your drive with a brand specific testing tool(or a third party one) as there may be something wrong with it(bad sectors for example).
