disk crashes while defragmenting

My disk keeps crashing when my pc tries to defragment the HDD, last time it was at the consolidating fase when it occured..

any ideas of what it could be?

(WD caviar blue 1TB 7200rpm)

do a scan disc and repair on start up......... how full is the drive?
