I am getting ready to install the EVGA 3090 Ti GPU and noticed the 16 pin to 3 x 8 pin adapter needed for plugging into the PSU. I noticed the end of the adapter just has the larger 12 pins but does not have the smaller 4 pin connector shown on the GPU. Am I missing something here or are there other cables that need to be connected. The instructions do not say.

The RTX 3090 Ti adapters only come with the 12 pin as they don't use the sense pins. It was a weird choice, but it is how they shipped them.
Thanks..yes it was weird. The directions said to plug in the 16 pin adapter in the GPU but the image only showed a 12 pin. So I guess the other 4 pins are not needed. I am assuming I use VGA extension cables for each to the PSU. I have an EVGA 1000 G5.

If the adapter came with the card, you should be good to go, though if I were you I'd also look into the PSU's end. What is the make and model of the unit you're working with?
The make of the GPU is an EVGA GeForce RX 3090 Ti plugging into an EVGA 1000 G5
