Eyefinity Active MDS-DVI Issue

Alright, so I'm trying to set up my four monitors and I believe the issue is with the Mini-DisplayPort to DVI Adapter.

Here's the adapter:

Setup:
#1 ==> MD Port --- MDP-HDMI Active Adapter --- HDMI Cable --- Monitor
#2 ==> DVI Port --- DVI Cable --- Monitor
#3 ==> HDMI --- HDMI Cable --- Monitor
#4 ==> MD Port --- MDP-DVI Active Adapter --- DVI Cable --- Monitor

Now the first three monitors work fine, but #4 does not. I tried switching the DVI cables so that #4 was a direct connection and #2 went through the adapter. The result was #1, #3, and #4 working, but #2 did not. Can someone confirm that the adapter is the issue? I just purchased it today, so I thought it might be faulty. Thank you.
