Failed RAM

I recently discovered that one of my RAM modules failed memtest86. It is brand new and was working fine for about a week, so my question is, was the RAM defective from the factory and just broke, or was it something the motherboard did (either some change in BIOS I may have done)?
 

I overclocked it by 48MHz (BCLK changes) but I kept the voltages the same, would that have been enough?
 


Possibly, but maybe not. You haven't even told us what ram it is or what settings your running it at, or even what cpu or motherboard you have, so any comments are simply speculation, there are no mindreaders here that I know of.
 

Here will this help? Corsair Vengeance 1600MHz DDR3 1.5V, BCLK at 103.00 so RAM is at 1648MHz. Intel Core i7 2600k. ASUS P8Z68-V PRO/GEN3.
 
You shouldn't have to overclock the ram to overclock your cpu.

Alvine is right, improper handling can damage the ram. You need to handle the memory by the edges only, only set it down in or on a anti-static bag, discharge yourself on some metal before picking it up, and make sure your motherboards capacitors are empty of electricity before installing or uninstalling ram, or any other component. (press power button a few times after unplugging and turning off psu)
 

When building the computer I wore an anti-static wrist band that was plugged into the grounding outlet. I always make sure the capacitors are empty before working on any part of my computer and I have the PS switch on off, but plugged in to ground the mobo. It was my first build so I made sure everything was in check, I was EXTREMELY careful. Plus, only one of the modules seems to be broken, I just tested them both individually. And the mobo overclocked itself when I changed to performance mode. My question simply was, is it from manufacturing, or was it the mobo that did it, just for future precautions.
 

Yeah, I tried with XMP settigns with BCLK at 100.00, it still fails. I get 2000+ errors in the first 10 minutes. Thanks anyway, and I guess I'll have to go and buy new RAM. I am left with only on module.
