Fallout NV Stutter even with anti stutter mod

I recently got Fallout NV on a steam offer (yea it´s an old game...)
anyway it has been running great the past 3 days and I´ve installed a few mods.
I did a clean install because an ENB I had installed was causing crashes, after the clean install with no ENB everything was fine but I started the Old World Blues DLC (the one in the circular map where -SPOILER- they remove your brain)
and now its laggy as F**k.
I´m getting 45-50 FPS but I can tell that there are big "gaps" in the FPS... it´s just realy laggy
I installed the NV stutter remover mod but it doesn´t do anything I´ve double checked that it´s installed right and I even loaded an earlyer save from when everything was fine at 60FPS but even those saves in diferent locations are realy laggy.
Any Ideas what this might be?

I have a R9 290 and it was running fine before... or is that DLC known for braking the game or something?

Reinstallation probably won't help unless you're moving it to another storage device. Mods are also known to bring Bethesda games to their knees. A ton of optimization was put into Skyrim (and it shows) but even that is known for having issues.
The Gamebyro engine is known for this. Stuttering was a huge problem in Oblivion, Fallout 3, and Fallout New Vegas. I do not know of any software fix which works really well, just a handful of config tweaks which may or may not break the game. Putting the game on an SSD helps tremendously though.
