Fatal Errors while playing games or randomly.

I've been playing a few games lately like MK X, GTA V, Orion: Prelude, etc. While playing MK X, my screen freezes up and I have to force shut down my computer, usually while playing GTA V, I encounter random fatal errors. I played Orion: Prelude for a while, then I quit it and a bit later, bam, fatal error.

I don't know what the problem is, is it my RAM? I've ruled out HDD as not being the problem. The fatal errors appeared on my 250GB HDD, and after attaching a new 1TB one, and separating the 250GB one, the problem still occurs. I'm unsure of what to do, it's just that before I buy something new I wanna make sure that it is exactly the problem. Can the RAM be a matter?
 
Your CPU is really hot - 74C. I gather you have a stock cooler on it. Blow the dust out of it. Try re-seating it (new thermal paste) or get a better cooler eg Cryorig H4.

Your Power Supply 12v rail is getting too low - 11.5V

Your GPU is getting too hot too - 77C. Your GPU can is running at 77%. Use After burner to make a custom fan profile to bump up the fan speed.

What GPU make and model?
 
It's the sapphire GhZ edition.

I also never had any problems in the past....Can a cooler really be the problem? And will it fix it? Because I wanna be sure. And what about the PSU....What can I do about it?
