Full disk problem

So the title pretty much describes my problem. While playing some games, for example Rainbow Six Siege, my disc is always at 95-100% and it's causing issues like crashing my game often and freezing. I don't know which component is the problem here. I have 8GB 1600mhz ram, I feel like that should be enough, so I think it may be my GPU. I have a Sapphire Nitro Radeon R9 380 2GB. I think this is the problem and doesn't have enough Vram. Or perhaps it could be my hard drive. I have a Seagate barracuda 1Gb 7200rpm. Rest of specs are CPU= AMD FX-6300. PSU = Coolermaster B6 600 watts. CPU cooler is coolermaster hyper t4. Thanks in advance.
 
Look at swap usage. IF game swaps and writes game data to HDD it will shutter and freeze like crazy. Not sure if it would crash, but this might be reason for freezes. IF you dont have enough Vram GPU will use system ram. and then swap (which is horible)
