FX 8350: Idles at max frequency and voltage

When Windows boots up, the process sits at its low frequency, and behaves as normal. However, a few minutes in, it ramps up its frequency and sits at 4110Mhz for the rest of its uptime, even in idle. The system sits at around 5-10% CPU usage, and yet the CPU is maxed out at all times

I've read tons of threads and tried many solutions. I've enabled all of the relevant BIOS settings (Cool'n'quiet, C6, C1, etc.), I've updated all of my drivers and BIOS, selected Balanced Plan in power settings, checked that nothing was using the CPU.

Any ideas as to what could be causing the issue?
 


Turning off Turbo core simply limits the frequency to 4012Mhz, and voltage down to 1.375v. It still sits on 4012Mhz idle, but now fluctuates when I open programs.
