The UBM stuff is useless in that format. The page link needs to be provided.
Include the make and model of the power supply(Ex: Corsair RM750X), and how long it has been in use.

Using hwinfo, what's the gpu core and gpu hot spot when in game?
Further down, there is Gpu Performance Limiters: Power, Thermal, Rel Voltage, MOp Voltage, Utilization - the SLI should be ignored. You may have to click the drop-down arrow next to it to see them all. It's normal for at least one to be reached, but Thermal is by far the worst one of the limiters to run into.

Hotspot : 50
GPU Core Load : 23 %
Core Voltage : 0.650
Performance Limit - Thermal : No
Performance Limit - Power : Yes

Im not sure if I provided the correct information I don't have much knowledge about these values sorry let me know if you need anymore.

Yes, I was hoping to see screenshots showing hwinfo with the requested information from a game session. [You must go through an imagehosting site to post pics here, otherwise nothing will appear when you try to post one.]

Make and model of the psu was left out, as well as how long it has been in use.

If this is any help ^
It doesn't show anything that would help diagnose the problem. Sorry.

Still missing: Make and model of the psu was left out, as well as how long it has been in use.

One or more sensors on your gpu appears to have failed. It's power throttling because the gpu believes it's using crap tons of power.
In the screenshot of hwinfo you provided, look at:
-Gpu Power
-Rail Powers
-Gpu Power[% of TDP]
-Gpu Power (normalized)
They're all unusually high.

In use since around 2020,
Make / Model of PSU Unknown. Do I need to take it apart to find out or is there another way?

How do I fix the issue?

If you don't know it, you will have to find the label on one of its sides.

You'll have to try a repair shop, or get a new gpu.
I was thinking the same, however it seems to randomly work fine I adjust some settings and it works however then it randomly stops working again. I feel like its fixable idk how, any suggestions I can try before taking it to one?
