Graphic card issue?

I just recently bought a new video card and power supply to fix the issue I'd been having with my computer. My issue is that graphics and sound break entirely and result in a BSOD or I have to manually restart my computer. I was hoping the new video card would have solved this issue but it turns out not. I had the nvidia gts 450 and I upgraded to an ASUS Nvidia gtx 960. Does anyone know what the issue is? The video is recent and it shows the problems I am having.
 


Biostar A770E3 for mobo, AMD athlon II x4 640 for cpu, Corsair CX500M for PSU. Everything is up to date.
 
Try powering down, uninstalling the GPU, then hook your monitor to the mobo I/O and reboot. Use the display driver uninstaller linked below (instructions on the download page), then power down, reinstall the card and reboot and do a fresh display driver install. Use a downloaded driver from the Asus support site, also linked below. If you get a pop-up asking if you want the hardware wizard to install the driver, click no and install it manually.
