Graphical problem please help

Hey guys, i've been having this weird graphical problem in games like gmod, and skyrim. It's been getting worse, i just bought skyrim and it's very un-enjoyable with this problem.... please help, and if you do find a solution please tell me how to fix it and what to do.

Those are the screenshots of whats happening,

could be a driver issue........... seek out a patch from whatever card ( amd/nvidia ) ( for skyrim ) and install it............... could be something as simple as a setting it the games control panel........... textures/shadows////// anything. you'll have to mess around until you find it.
